Differences Between Healthcare Solutions

There are multiple care solutions in the healthcare industry. Assisted Living, Home Care, Post Acute Care, and Population Health though different all have the same goal in mind which is improving health and the quality of life. These solutions provide holistic care, clinical programs, and other health services that are designed to aid in the safety of our loved ones. Read more to learn more about these care solutions. 

Assisted Living 

Assisted living communities are a great place for seniors to receive personalized care in a comforting residential space. Seniors whose well-being needs extra support stay in assisted living communities that offer more than just caregivers but opportunities to be social, live a healthy lifestyle, exercise classes, art classes, and therapies. Other services provided include transportation services, 24/7 available staffing, meals, housekeeping, laundry, grooming, bathing, dressing and more.

Home care is a type of care that is able to be given within the comfort of your own home. With home care caregivers help patients in their homes to complete daily tasks to maintain their independence. Post Acute Care 

Post Acute Care is care given after a patient has returned from the hospital from injury or illness and needs continued medical care. At Spring Hills there are various medical care services provided when supporting the recovery of a patient including, stroke rehabilitation, wound care, hospice and palliative care, parkinson’s and neurological care, memory care and more

Population health is the health outcomes of a group of individuals, including the distribution of such outcomes within the group. It’s approach is aimed to improve health and consists of three components, health outcomes, patterns of health determinants, and policies and interventions. It is about identifying patients’ needs, and giving a smooth transition back to your home. Population health programs consist of making sure patients are set up to manage their health on their own post care.
