Spring Hills Post Acute Matawan

A Strong Support System is Key to Happiness

At Spring Hills we aim to give patients the support and resources they need to live healthier lives. Fully integrated with our clinical programs, patients and their caregivers are engaged by our multidisciplinary care team before hospital discharge to facilitate their transition to Spring Hills. Our associates ensure that the patient and caregivers are supported during their stay and for at least 90 days after they return home.

Signature Touches | The Essence of our Approach to Living

Our holistic approach is the cornerstone of our services that we call Signature Touches. Our personalized technique starts from the moment we meet as we discover more about you and your family. Focusing on specific needs and desires, allows us to customize care that enhances quality of life and engage patients in new experiences. Memory Care | Living with Dignity + Purpose

The essence of our philosophy to dementia programming is to provide person centered care. Our innovative memory care program is based on the strengths and interests of our residents, providing the ability to choose, maintain a sense of dignity, value, and purpose. We provide supportive and open communication with our families. All Care Partners are trained in Alzheimer’s and dementia care, focusing on caring with a commitment to quality and exceeding the expectations of our residents and their loved ones.

This Is The Moment That Matters

Safety is essential to the wellbeing of our memory care residents and peace of mind for those that love them. We take a proactive and respectful approach using technology to:

  • Provide real-time data
  • Increase efficiency for more one-on-one time
  • Connect with other healthcare providers

Care Flowers is a communication tool to understanding residents needs and appreciating what they may be trying to share. Different flowers represent certain wishes, thoughts, and feelings to help support a resident’s choice, independence and engagement efforts.

Post Acute Care | A Patient’s Journey to Independence

Our team makes every effort to set patients on the right course to recovery. Through our proven health management process, we help ensure they stay on track. We provide all of the premier care services you’d expect from a leader, including the very latest connective technology. Nothing will ever replace the power of holding someone’s hand and letting them know it’s going to be okay. That’s the Spring Hills difference.

Caring to Exceed Expectations

We have learned something in our 20 years of senior care experience: people respond to people. Our associates understand that being attentive to an individuals likes and dislikes is important for their happiness, and happiness is key to the health of body, mind and spirit. This is the essence of Signature Touches, our holistic approach to living. Our clinicians know that the more they engage patients face-to-face, the more that patient feels cared for and accountable to following their recovery plan. Building trust through personal relationships with the people in our care and their families is at the heart of all we do at Spring Hills, and exceeding expectations is our joy and privilege.

Our continuum of care combines proactive clinical care and personalized support at every
stage of an individual’s health care journey. Our multidisciplinary care team ensures that every critical transition, such as the move from hospital to home or the move from home to a long-term care residence, is safe and seamless.

“I have been a resident here for over 8 years and during this period I’ve had several serious health crises in which the staff amazingly helped me pull through. Upon return from the hospital, I required extensive nursing care which the nurses provided to me on a 24/7 basis. The nurses here are caring, passionate and have a full understanding of each and every resident here that has specific needs. I would highly recommend this facility.”
