The essence of our philosophy to dementia programming is to provide person centered care. Our innovative memory care program is based on the strengths and interests of our residents, providing the ability to choose, maintain a sense of dignity, value, and purpose. We provide supportive and open communication with our families. All Care Partners are trained in Alzheimer’s and dementia care, focusing on caring with a commitment to quality and exceeding the expectations of our residents and their loved ones.

Care Flowers is a communication tool to understanding residents needs and appreciating what they may be trying to share. Different flowers represent certain wishes, thoughts, and feelings to help support a resident’s choice, independence and engagement efforts.

Home Care | Private, Personalized Care in the Comfort of Your Home

Not ready to make the big move? We understand. For seniors whose happiness is being at home, our Home Care Services provide flexibility, personalized support and the assurance of safety. Our dedicated certified Home Health Aides (CHHAs) are trained to care for individuals with chronic illnesses and conditions such as dementia, cardiovascular disease, and osteoporosis. We provide condition-specific education, exercise programs, and other assistance to help individuals manage their condition while living where they feel best. If the big move is causing added anxiety, ask us about our Welcome Home Program.

Poet's Walk San Antonio had a Case Managers Appreciation Lunch with Methodist Hospital, Elysian Hospice, Advanced Healthcare, & TheraCare! We provided lunch and a quick presentation to 65 case managers. Everyone was given lunch and PWSA trifold. It was a great turn out!

As an employee at this memory care facility, I feel that the best quality that is apparent is the outpouring of love that I see from all departments toward our residents. It's one thing to "just work here" versus the feeling of community that I see on a daily basis. I can't say enough about Poet's Walk!

Dissatisfied with Mom's prior residence... we found Poet's Walk in Warrenton Va. And instantly Carol Rose made our Mom feel like she was home. She has been an Incredible conduit between our Mom and our crazy hectic world! She has been instrumental in keeping the residents safe during these difficult times. We do feel like Terry at the front desk, Jodi, Iro and Tracy are family.We highly recommend this Home!

I’ve been working here since the start of 2021. This is hands down the best team that I’ve worked with and been a part of! It’s a very comfortable and inviting atmosphere to be in. Every department is filled with genuinely kind people which makes working here just that much better. Managers truly care about how each and every worker feels and is doing and about the staffs thoughts on how things could be improved or done differently. Plus working here is very rewarding and if you’re ever given the chance to work here do it especially if you want to make a difference and have the willingness to commit to the care and well being of not only the resident but the family members!
